Treatment note templates

Build the structure of your treatment notes — the sections and fields you control — and bind each template to a billable service so the right form loads automatically.

AI summaryTL;DR
  • A treatment note template (form structure) is the layout of a note — the sections and fields you fill in during a visit.
  • Build your own: add, remove, reorder and rename sections and fields, or start from a ready-made layout with Load Template.
  • Bind a template to a billable service and it loads automatically whenever you write a note for that service.

Every clinic writes notes a little differently. Treatment note templates let you shape the note to the way you work — which sections appear, what fields they hold, and the order they’re in — instead of bending your notes to fit a fixed form. Build a template once, bind it to the services that should use it, and it’s there every time you open a note.

Where templates live

Open Treatment Form Structures to see every template in your clinic. Each one shows its name, an active/inactive switch, and the billable services it’s bound to. From here you can create a new template or open an existing one to edit.

Treatment Form Structures — the list of templates with their bound services

Build or edit a template

Open a template — or choose Create Treatment Form Structure — and you’ll work through three parts:

  1. 1
    Treatment form details

Give the template a clear Name and Description, and set it Active so it’s available to use. Inactive templates are kept but hidden from the picker.

  2. 2
    Form structure contents

The builder where you lay out the note itself (see below). Use Load Template to start from a ready-made layout, or build on a blank canvas.

  3. 3
    Billable service bindings

Choose which services this template is used for — covered under “Bind a template to a service” below.

Add, reorder and remove fields

Inside the builder, the note is made of blocks. Hover over any block to reveal its controls:

  1. 1
    Add a block

Hover and click the + button, or type / anywhere to open the block menu, then pick what to add.

  2. 2
    Reorder

Grab the drag handle that appears on the left and drag a block up or down. Drop it where you want it.

  3. 3
    Duplicate or delete

Open the drag-handle menu to duplicate a block or remove it.

  4. 4
    Configure a field

Text fields have a settings menu for the label, helper text, number of rows, and whether the box accepts numbers only.

Form builder — a section with fields, drag handles and the add-block menu

You can build a note from a range of blocks: sections to group related fields, headings and paragraphs for structure and instructions, and fillable fields — text (single or multi-line, or numbers only), checkboxes and checkbox groups, radio options, toggles, a slider, date and time pickers, and a signature box.

Bind a template to a service

Binding is what makes a template load by itself. In Billable service bindings, pick the services that should use this template — for example, your initial physio consult. From then on, whenever you write a note for an appointment with that service, this template opens ready to fill in.

  1. 1
    Open Billable service bindings

Inside the template, go to the bindings section.

  2. 2
    Add the services

Select each billable service that should use this template. They move into the bound list.

  3. 3
    Save

Click Save Form Structure. The binding takes effect immediately for new notes.

Use a template while writing

When you open a treatment note, the template bound to that appointment’s service loads automatically — there’s nothing to pick. Want a different layout for this one note? Open the template picker in the note, search by name, preview a template and choose Use This Template.

Frequently asked

Yes — templates live at the clinic level, so the team shares them. Binding a template to a service sets it for everyone who writes notes for that service.

Your templates are the form structures you build and bind to services. Load Template is a library of ready-made starting points you can drop into the builder and then tailor.

Any time. Changes apply to new notes that use it; notes you’ve already written keep what you saved.
