AI summaryTL;DR
  • A patient record gathers personal, contact, address, emergency-contact and employment details in one place.
  • Attach files such as signed consent forms directly to the patient.

Personal details

Who the patient is. The basics drive ACC claims, reminders and reporting:

  1. 1
    Name & title

First name and last name (required), plus an optional title.

  2. 2
    Date of birth & gender

Used across claims and clinical context.

  3. 3
    NHI number

The patient’s National Health Index number, e.g. ABC1234.

  4. 4
Date of first visit & ethnicity

    Recorded for reporting and ACC.

Contact

How you reach the patient. Add one or more phone numbers and mark a preferred number, plus an email. The SMS consent toggle controls whether reminders can go out by text.

Address

Search for an address, or enter it manually if it isn’t found.

Emergency contact

A name, relationship and phone number for someone to reach in an emergency.

Employment

The patient’s occupation and whether they’re in paid employment — relevant to ACC claims.

Attachments

You can attach files directly to a patient — most often a signed consent form, but any document that belongs to the person rather than a specific injury.
